Spironolactone vs Tretinoin for Acne: Inside Out or Outside In

Tretinoin works on the skin. Spironolactone works on the hormone driving the breakout. If your acne sits along the jawline and ignores every cream you own, that distinction is the answer.

These treat the same visible problem from opposite directions. Choosing between them starts with where your acne appears and how it behaves, not with which is stronger.

Tretinoin works on turnover

Tretinoin is a topical retinoid. It accelerates skin cell turnover so pores clear rather than plug, and over time it stimulates collagen, which is why the same molecule treats acne and fine lines.

It is the best evidenced topical in dermatology and the benchmark every over-the-counter retinol is measured against. It works particularly well on comedonal acne, the blackheads and small bumps across the forehead and cheeks.

Spironolactone works on hormones

Spironolactone blocks androgen activity in the skin. Androgens drive sebum production, and excess sebum drives the deep, tender, cystic acne that appears along the jaw and chin, often flaring on a monthly cycle.

That kind of acne rarely responds to topicals, because the driver is systemic and a cream cannot reach it. This is the single most useful thing to understand about hormonal acne.

How to tell which you have

  • Jawline, chin, and neck, deep and tender, cycles monthly: hormonal, consider spironolactone.
  • Forehead, cheeks, small bumps and blackheads: comedonal, start with tretinoin.
  • Both patterns at once, which is common: the two are frequently prescribed together.

They combine well

Because the mechanisms do not overlap, using both is common and sensible. Spironolactone reduces the hormonal driver while tretinoin keeps pores clear, and neither interferes with the other.

The practical differences

Tretinoin causes purging and flaking through weeks two to six, which is unpleasant and temporary. Starting twice weekly and building up reduces it considerably. Daily sunscreen is not optional on tretinoin.

Spironolactone requires reliable contraception, because it is unsafe in pregnancy. Your clinician will usually check potassium before starting and periodically after. Irregular periods and breast tenderness are common in the first months.

Timelines

Tretinoin shows meaningful improvement around month three, with continued gains through month six. Spironolactone typically takes two to three months to show its full effect.

Neither is fast. Both are far more durable than the antibiotic course that clears you in a fortnight and stops working by spring.
